GUIDELINES FOR NOMINATIONS
Nominations may be submitted electronically or by mail.
Submissions must include the nominee’s name, title, address, phone number and email address as well as the name and contact information for the primary nominator and supporting nominator (if applicable).
Nominations must include a narrative that briefly explains how the nominee meets the criteria for the particular award and why the person qualifies for this honor.
Submissions may also include the nominee’s resume and up to two additional letters of support at the nominator’s discretion.
Each nomination should also include the category to which the person is being nominated.
